If your income exceeds certain thresholds, you’ll pay more than the standard Medicare Part B and Part D premiums. This surcharge is called IRMAA — the Income-Related Monthly Adjustment Amount. Here’s how it works for 2027.
What Is IRMAA?
IRMAA is an additional monthly charge added to your Part B and Part D premiums if your Modified Adjusted Gross Income (MAGI) from two years prior exceeds the threshold. In 2027, Social Security uses your 2025 tax return to determine your IRMAA.
2027 IRMAA Part B Brackets
| Individual Income (2025) | Joint Income (2025) | Monthly Part B Premium |
|---|---|---|
| ≤$106,000 | ≤$212,000 | $185.00 (standard) |
| $106,001–$133,000 | $212,001–$266,000 | ~$259.00 |
| $133,001–$167,000 | $266,001–$334,000 | ~$370.00 |
| $167,001–$200,000 | $334,001–$400,000 | ~$480.00 |
| $200,001–$500,000 | $400,001–$750,000 | ~$591.00 |
| >$500,000 | >$750,000 | ~$628.00 |
Note: Exact 2027 brackets are set by CMS and adjusted annually for inflation.
Can You Appeal IRMAA?
Yes. If your income dropped significantly due to a life-changing event — retirement, divorce, death of spouse, loss of income-producing property — you can file a Social Security Life Event Appeal using a more recent tax year. Common qualifying events:
- Retirement or reduction in work hours
- Death of a spouse
- Divorce or annulment
- Loss of pension income
- Loss of income-producing property (e.g., due to disaster)
IRMAA and Medicare Advantage
IRMAA applies to your Part B premium regardless of whether you’re on Original Medicare or Medicare Advantage. You’ll still pay the surcharge on top of your Part B payment.
