One of the most common questions during AEP: “Is the $0 premium plan too good to be true?” The answer is: not necessarily — but there are tradeoffs worth understanding before you enroll.
How Can a Medicare Advantage Plan Cost $0?
Insurance companies receive a monthly payment from Medicare for each enrolled member. When that payment exceeds the cost of administering the plan, the insurer can offer a $0 premium while still making a profit. This is most common in high-density areas where competition is strong.
Important: you still pay your Part B premium ($185/month in 2026) even with a $0 plan. “Zero premium” refers to the plan’s additional charge on top of Part B.
What’s Typically Included in $0 Plans
- All Medicare Part A and Part B benefits
- Prescription drug coverage (MAPD)
- Dental, vision, and hearing benefits
- Fitness membership (SilverSneakers or similar)
- Over-the-counter (OTC) allowance
- Transportation benefits (some plans)
What to Watch Out For
- Network restrictions: HMO plans require you to use in-network providers. If your doctor isn’t in the network, you’ll pay full cost.
- Prior authorizations: Many MA plans require approval before procedures, specialist visits, or certain medications.
- Out-of-pocket maximums: A $0 plan may have a higher OOP max than a plan with a small premium.
- Drug formulary: Check that your specific prescriptions are covered at an affordable tier.
- Dental fine print: “$2,000 dental benefit” often has waiting periods, limits on covered procedures, or applies only to in-network dentists.
$0 Plan vs. Plan With Premium: Quick Example
| Factor | $0 Premium Plan | $45/mo Premium Plan |
|---|---|---|
| Monthly cost | $185 (Part B only) | $230 ($185 + $45) |
| Annual OOP max | $8,500 | $4,000 |
| Network type | HMO | PPO |
In this example, someone with significant medical needs may actually save money on the $45/month plan due to the lower out-of-pocket maximum.
