Understanding Your ACA Plan’s Deductible, Copay, and Out-of-Pocket Maximum

Health insurance has its own language, and misunderstanding a few key terms can cost you real money. Here’s a plain-English explanation of the three most important cost-sharing concepts in your individual health plan.

Deductible

The deductible is the amount you pay out-of-pocket for covered services before your insurance starts sharing costs. If your deductible is $3,000, you pay the first $3,000 of covered medical expenses each year yourself.

Important nuance: Many plans have separate deductibles for medical services and prescription drugs. And most plans cover preventive care at 100% with no deductible — your annual physical, screenings, and certain vaccines are free regardless of whether you’ve met your deductible.

Copay

A copay is a fixed amount you pay for a specific service — like $30 for a primary care visit or $50 for a specialist — regardless of your deductible status. Some plans apply copays before the deductible is met; others apply them only after. Read your plan’s Summary of Benefits to know which applies to you.

Coinsurance

After meeting your deductible, coinsurance kicks in. This is a percentage split — typically 80/20 or 70/30 — where the insurance company pays the larger share and you pay the smaller one. On a $10,000 covered procedure with an 80/20 plan (after deductible), you’d pay $2,000.

Out-of-Pocket Maximum

The out-of-pocket maximum is your safety net. It’s the most you’ll ever pay in a plan year for covered services. Once you hit this limit, insurance covers 100% of covered costs for the rest of the year. The ACA caps individual out-of-pocket maximums at $9,450 in 2026.

How These Work Together

Example: $2,500 deductible, 80/20 coinsurance, $7,000 out-of-pocket max.

  1. You pay the first $2,500 entirely
  2. Then you pay 20% of costs until total out-of-pocket hits $7,000
  3. After that, insurance covers 100% for the rest of the year
