Medicare Advantage vs. Original Medicare in 2027: Which Is Right for You?

By Paul Z Olah  |  October 19, 2026

Every year during AEP, Medicare beneficiaries face the same core question: should I stay on Original Medicare or switch to Medicare Advantage? There’s no universal answer — the right choice depends on your health, finances, and where you live.

How Original Medicare Works

Original Medicare is the federal program consisting of:

  • Part A — Hospital coverage (most people pay $0 premium)
  • Part B — Medical coverage ($185/month standard premium in 2026)

With Original Medicare you can see any doctor who accepts Medicare — no networks, no referrals. But there’s no cap on out-of-pocket costs, which is why most people add a Medigap plan and a Part D drug plan.

How Medicare Advantage Works

Medicare Advantage (Part C) is offered by private insurers approved by Medicare. These plans bundle Parts A and B and usually include Part D. Many have:

  • $0 premium (you still pay your Part B premium)
  • Annual out-of-pocket maximums (Original Medicare has none)
  • Extra benefits: dental, vision, hearing, fitness memberships
  • Network restrictions (HMO or PPO)

Side-by-Side Comparison

When Medicare Advantage Makes More Sense

  • You stay local and your preferred doctors are in-network
  • You want an out-of-pocket cap for financial protection
