How to Add a New Baby or Dependent During 2027 Open Enrollment

By Paul Z Olah  |  October 3, 2026

Adding a new family member to your health insurance plan is one of the most important coverage tasks new parents and growing families face. Here’s everything you need to know about adding dependents during 2027 Open Enrollment and throughout the year.

Adding a Dependent During Open Enrollment

During Open Enrollment (November 1, 2026 – January 15, 2027), you can add any eligible dependent to your marketplace plan as part of your annual enrollment. Eligible dependents include:

  • Spouse or domestic partner (depending on state rules)
  • Children up to age 26 (regardless of marital status, student status, or financial independence)
  • Dependent parents or other relatives in some circumstances

Simply include them when you complete your enrollment application. The household size affects your premium tax credit calculation — more dependents = higher FPL threshold = potentially more subsidy.

Adding a Newborn Mid-Year (Special Enrollment)

Birth triggers a Special Enrollment Period — you have 60 days from the date of birth to add the newborn to your plan. Don’t wait. Most plans also retroactively cover the newborn from the date of birth if added within 30 days, so early action protects against any birth-related claims falling through.

Steps to add a newborn:

  1. Log into your marketplace account
  2. Select “Report a life change” and indicate the birth
  3. Add the newborn’s information
  4. Select or confirm your plan
  5. Pay any updated premium

Adding an Adopted Child

Adoption and foster care placement also trigger SEPs. Document the adoption or placement date — you’ll typically have 60 days from the legal placement to enroll the child. Some marketplace rules cover the child from the placement date regardless of when you formally enroll, similar to birth coverage rules.

Turning 26: Aging Off a Parent’s Plan

Children covered under a parent’s marketplace plan lose eligibility when they turn 26. This is a qualifying life event that triggers a 60-day SEP for the young adult to enroll in their own plan. If your child is approaching 26 in 2027, help them plan their enrollment during the Open Enrollment window if possible — it’s cleaner than a mid-year SEP.

How Dependents Affect Your Subsidy

Each additional household member increases your FPL threshold, which can meaningfully increase your premium tax credit eligibility. A family of 3 can earn roughly $15,000–$20,000 more than a family of 2 and still qualify for the same subsidy percentage. Adding a new family member mid-year may also change your tax credit — update your marketplace application accordingly.
