Health Insurance for Families During 2027 Open Enrollment: A Complete Guide

By Paul Z Olah  |  September 28, 2026

Choosing health insurance for your whole family is one of the most important financial decisions you’ll make during 2027 Open Enrollment. The window runs from November 1, 2026 through January 15, 2027. Here’s how to approach it as a family.

Start With Your Family’s Healthcare Profile

Before comparing plans, take inventory of what your family actually needs:

  • How many people are on the plan, and what are their ages?
  • Does anyone have a chronic condition or take regular prescriptions?
  • Is anyone pregnant, or is a pregnancy planned for 2027?
  • Do your children need specialist care, orthodontics, or ongoing therapy?
  • Are your current doctors in-network on the plans you’re considering?

A family with several young children and a parent with a chronic condition will need a different plan than a family of four where everyone is healthy.

Family Deductibles: Individual vs. Embedded

Family plans typically have two deductible structures:

  • Embedded deductible: Each family member has their own individual deductible, plus a separate family deductible cap. One person maxing out their individual deductible doesn’t require the entire family deductible to be met first.
  • Aggregate deductible: The whole family shares one combined deductible. Benefits for any family member don’t kick in until the aggregate is met — which can be a significant financial exposure for larger families.

For families with children who use healthcare regularly, an embedded deductible is often more protective.

Pediatric Coverage Is Mandatory

ACA marketplace plans are required to cover pediatric dental and vision as part of the essential health benefits. However, the level of coverage varies. If your children need more comprehensive dental or vision care, you may want to supplement with a standalone pediatric dental or vision plan.

Maternity and Newborn Coverage

All marketplace plans must cover maternity care and newborn services. If you’re planning a pregnancy in 2027, this is already built in — but review how the specific plan handles prenatal visits, hospital delivery, and postpartum care, as cost-sharing varies.

Adding a New Baby During the Year

Birth, adoption, or placement for adoption triggers a Special Enrollment Period. You have 60 days from the event to add the new family member to your plan. Newborns are typically covered from birth if added within 30 days — but confirm the exact window with your insurer.

Subsidy Eligibility for Families

Premium tax credits are calculated based on household income relative to the Federal Poverty Level for your family size. Larger families have a higher FPL threshold, meaning a family of four can earn significantly more than a single person and still qualify for meaningful subsidies. When comparing plans, always compare the after-subsidy premium — not the full sticker price.
