Every year during AEP, Medicare beneficiaries face the same core question: should I stay on Original Medicare or switch to Medicare Advantage? There’s no universal answer — the right choice depends on your health, finances, and where you live.
How Original Medicare Works
Original Medicare is the federal program consisting of:
- Part A — Hospital coverage (most people pay $0 premium)
- Part B — Medical coverage ($185/month standard premium in 2026)
With Original Medicare you can see any doctor who accepts Medicare — no networks, no referrals. But there’s no cap on out-of-pocket costs, which is why most people add a Medigap plan and a Part D drug plan.
How Medicare Advantage Works
Medicare Advantage (Part C) is offered by private insurers approved by Medicare. These plans bundle Parts A and B and usually include Part D. Many have:
- $0 premium (you still pay your Part B premium)
- Annual out-of-pocket maximums (Original Medicare has none)
- Extra benefits: dental, vision, hearing, fitness memberships
- Network restrictions (HMO or PPO)
Side-by-Side Comparison
| Feature | Original Medicare | Medicare Advantage |
|---|---|---|
| Monthly premium | Part B only (~$185) | Part B + plan premium (often $0) |
| Out-of-pocket max | No cap | Capped (varies by plan) |
| Doctor choice | Any Medicare doctor | Network only (HMO/PPO) |
| Drug coverage | Requires separate Part D | Usually included (MAPD) |
| Extra benefits | None | Dental, vision, hearing, more |
| Referrals needed | No | Depends on plan type |
