Many people shopping the ACA marketplace focus exclusively on medical plans — and then are surprised to discover their new health plan doesn’t cover dental or vision. Here’s how dental and vision coverage works for individuals shopping on their own.
ACA Marketplaces and Dental Coverage
The ACA requires marketplace plans to cover dental benefits for children (under 19) as an essential health benefit. Adult dental coverage, however, is optional — many medical plans don’t include it at all.
The marketplace offers standalone dental plans (called “dental-only” or “SADP” plans) that can be purchased alongside or separately from a medical plan. These are the main way adult individuals get dental coverage through the marketplace.
Types of Dental Plans Available
- DPPO (Dental PPO): Most flexible; allows you to see any dentist, with better coverage for in-network providers. Most commonly purchased type.
- DHMO (Dental HMO): Requires a primary dentist and referrals; usually lower premium but less flexibility.
- Indemnity plans: Pay a set amount for each service regardless of provider; maximum flexibility but often higher out-of-pocket.
What Dental Plans Typically Cover
- Preventive (100% covered): Cleanings, X-rays, exams
- Basic (50–80% covered after deductible): Fillings, extractions
- Major (50% covered after deductible): Crowns, root canals, dentures
- Annual maximum: Most individual plans cap benefits at $1,000–$2,000/year
Vision Coverage
Vision coverage for adults is even more limited in the ACA framework. Marketplace medical plans generally don’t cover routine vision. Standalone vision plans are available both on and off the marketplace, and through independent carriers. These typically cover annual exams and an allowance for frames or contacts.
Off-Marketplace Options
Dental and vision plans purchased off-marketplace can sometimes offer better value than marketplace-only options. An independent broker can compare both on- and off-marketplace dental/vision options for your area.
